Home Care Packages

Individualised services to support Elders to stay healthy and active at home and in the community.

About the program

Aboriginal Community Services offer services to assist Elders with aged care services and supports, including everyday wellness, household tasks and social engagement to keep them involved in the community.

Home Care Packages are delivered through a consumer directed care model, where the Elder has the choice and flexibility on how care and support is provided.  They are designed for Elders whose aged care supports go beyond what is available through the Commonwealth Home Support Programme.

We will coordinate individualised services to support you to stay healthy and active at home and in the community.

In consultation with you, we will discuss your care needs, goals, wellbeing and your Home Care Package budget to coordinate services and supports to meet your aged care needs.

We support Elders with Home Care Packages across South Australia.

About the levels of care

A Home Care Package is Government-subsidised individual funding for an Elder to use on aged care services and supports.

There are four levels of Home Care Packages to support Elders with varying needs.

Level 1 - Basic Care Needs

Level 2 - Low Care Needs

Level 3 - Intermediate Care Needs

Level 4 - High Care Needs

Your package level is determined through a comprehensive assessment.

How to get services

Aboriginal Community Services delivers care and support services to people who identify as Aboriginal and/or Torres Strait Islander.  

To receive Home Care Package services through Aboriginal Community Services:

Contact My Aged Care

Register with My Aged Care either by telephone on 1800 200 422 or online at My Aged Care and request an assessment.

My Aged Care will ask you a series of screening questions to determine your needs and assess your eligibility for an aged care assessment.  If you are eligible for aged care assessment, My Aged Care will confirm your contact details and refer you to an assessor.  For Home Care Packages, this will be your local Aged Care Assessment Team (ACAT).

Get Assessed for a Home Care Package

A member of an ACAT will contact you for intake, arranging a suitable time for you to complete the Comprehensive Assessment.  

The comprehensive assessment takes into consideration your overall wellbeing and aged care needs.  Your ACAT assessor will determine your aged care approvals, including your approved Home Care Package level.  The results of your comprehensive assessment will be sent to you via your preferred option as agreed at your assessment.

Receive Your Letter of Approval

My Aged Care will send you the result of your comprehensive assessment, which will set out the level of Home Care Package you are approved to receive.  You will also be placed on the national waitlist for a Home Care Package.  

Wait times vary; there may be a waiting period between the time you are approved for care and the time you are assigned a Home Care Package.  

If you don’t receive an approval letter explaining your assessment outcome, call My Aged Care to request a copy.

Receive Your Unique Referral Code

When a Home Care Package becomes available, you will be sent a letter from My Aged Care to let you know that you have been assigned a package. The letter will set out the level of Home Care Package that has been assigned to you and include a unique referral code to provide to your service provider of choice.

Take Up Your Home Care Package with Aboriginal Community Services

Aboriginal Community Services will work with you to design a care plan based on your needs, goals, and Home Care Package budget.  You will be provided with a Home Care Agreement for signing.

We care for your wellbeing

We offer services to assist Elders with health needs, everyday wellness, household tasks and social interactions to keep them involved in the community. Each Elder can design their individual support service plan to meet their specific needs and preferences. Services may include (but are not limited to):

What Services Can I Access With a Home care package?

Mobility and Dexterity

Providing crutches, quadruped walkers, walking frames, walking sticks, mechanical devices for lifting, bed rails, slide sheets, sheepskins, tri-pillows, pressure relieving mattresses and assistance with the use of these aids.

Continence Management

Assistance in using continence aids and appliances such as disposable pads and absorbent pads, commode chairs, bedpans and urinals, catheter and urinary draining appliances, and enemas.

Nursing, Allied Health and Other Clinical Services

Speech therapy, podiatry, occupational or physiotherapy services, hearing and vision services.

Nutrition, Hydration, Meal Preparation and Diet

Assistance with preparing meals, using eating utensils and feeding.

Transport and Personal Assistance

Assistance with shopping, visiting health practitioners and attending social activities.

Personal care

Assistance with personal activities such as bathing, showering, toileting, dressing and undressing, mobility and communication.

Management of Skin Integrity

Assistance with bandages, dressing and skin emollients.

Garden Maintenance/Lawn Mowing
