Commonwealth Home Support Programme

Helping Aboriginal Elders live independently at home, maintain quality of life and to enjoy community and cultural activities.

About the program

The Commonwealth Home Support Programme provides entry level care into Australia’s aged care system.

CHSP is designed to provide relatively small amounts of a single or a few services to support Elders in maintaining independent community living and wellbeing.

Aboriginal Community Services is committed to assisting Elders to maintain their independence at home and in the community, and to reduce early admission to residential care. 

We offer a range of in-home and social support services for Elders across Adelaide.

Services we can offer you

Assisting Elders to maintain their living areas.  This involves assistance to accomplish your daily chores in a safe, secure and healthy home environment.

At Aboriginal Community Services, we aim to provide you with the highest quality service possible that responds to your aged care needs while respecting your cultural wellbeing and assisting you to remain independent. 

Our Commonwealth Home Support Programme is a culturally safe, responsive and respectful service that is available to Elders across Adelaide.

We encourage Elders to participate in community life through structured, group-based activities that develop, maintain or support independent living and social interaction.

How to get services

Aboriginal Community Services delivers care and support services to people who identify as Aboriginal and/or Torres Strait Islander.

To receive services under the Commonwealth Home Support Programme through Aboriginal Community Services:

Contact My Aged Care

Register with My Aged Care either by telephone on 1800 200 422 or online at My Aged Care and request an assessment.

My Aged Care will ask you a series of screening questions to determine your needs and assess your eligibility for an aged care assessment. If you are eligible for aged care assessment, My Aged Care will confirm your contact details and refer you to an assessor. For the Commonwealth Home Support Programme, this will be your local Regional Assessment Service (RAS).

Get Assessed for the Commonwealth Home Support Programme

A member of a RAS will contact you for intake, arranging a suitable time for you to complete the Home Support Assessment.

The Home Support Assessment takes into consideration your goals, daily routines, lifestyle and overall wellbeing. The results of your home support assessment and eligibility for services will be discussed with you at your assessment.

For each CHSP service you are eligible for there will be a unique referral code to provide to your service provider of choice. You can request that your assessor refer your eligible CHSP services to Aboriginal Community Services or you can contact us to discuss your care options.

Commencing CHSP Services with Aboriginal Community Services

Aboriginal Community Services will work with you to design a care plan based on your eligible CHSP services. You will be provided with a Commonwealth Home Support Programme Agreement for signing.

The Agreement set out the services that will be provided to you and the basis upon which they will be delivered.
