About the role

The Transport Coordinator will report to the General Manager Client Services and provide effective coordination of client transport and individual services. You will work with a close knit team of professionals to promote and achieve positive client outcomes.

About you

To be successful you will possess a natural ability to communicate and actively engage clients in meaningful and positive manner. Your strong, resilience, attention to detail and accuracy and ability to work in a fast-paced environment will see you succeed in this role. Previous experience working with Aboriginal people be highly regarded but not essential. You must be willing to handle a very high volume of daily inbound & outbound calls with Elders/Clients in an aged care setting. You must possess unrestricted work rights in Australia. You must have an unrestricted driver’s license & a recent national police check. Responsibilities
  • Plan, schedule, and coordinate all transport bookings for individual clients, group activities, community programs, and major events, including completing client requests for medical transport, exercise group transport, social transport, and special requests.
  • Schedule exercise group bookings including contacting clients for each group, recording attendance, and booking transport as required.
  • Retrieve and provide daily reports of transport usage.
  • Engage with external transport agencies to provide a quality service with positive outcomes for clients.
  • Timely problem solving to ensure clients are provided with an efficient service.
  • Engage with co-workers to ensure transport services are delivered within clients identified care needs, goals and within service funding provisions.
  • Awareness of risk in a community setting, develop strategies and intervention to safeguard clients. Develop and maintain community service resources/relationships to maximise service provision and support clients.
  • Provide training and support to new staff.
  • Incidents (SIRS Reporting), feedback, complaints and improvements reporting to be created in an accurate and timely manner and in accordance with legislation, ACS policies and procedures.
  • Undertaking duties where delegated by the General Manager, Client Services.
  • Comply with all ACS Policy and Procedures and Perform all tasks and responsibilities to the level required.
