Community Programs Coordinator
About the role
This is a fixed-term contract of one year, with the potential to become ongoing based on an assessment towards the end of how this position has benefited the Elders/Clients.
The Community Programs Coordinator is responsible for the development, implementation and coordination of community based programs, social activities and assisting with major events for clients. This includes but is not limited to regular social activities; ongoing or once off cultural activities/events, wellbeing and lifestyle projects with the aim of enhancing involvement in the community, reducing social isolation, improving social and emotional well being of clients and assisting clients to participate in stimulating and therapeutic activities. The Coordinator is also involved in identifying opportunities for social wellbeing activities for clients with disabilities, challenging behaviours and those at risk of social isolation.
This position will work closely with the General Manager Metro Client Services and the Adelaide Metro client services team as well as support the General Manager Service Development with major event planning and coordination as required.

About you
To be successful you will possess a natural ability to communicate and actively engage clients in meaningful and positive manner. Your strong, resilience, attention to detail and accuracy and ability to work in a fast-paced environment will see you succeed in this role. Previous experience working with Aboriginal people be highly regarded but not essential. You must bring prior relevant experience as a Community Programs Coordinator for Elders/Clients in an aged care setting. You must possess unrestricted work rights in Australia. You must have an unrestricted driver’s license & a recent national police check.

Responsibilities
- Develop culturally safe social programs/activities that are inclusive to all Elders across the Adelaide Metropolitan aged care planning regions
- Develop financially sustainable social programs in line with budgeting requirements and clients HCP and CHSP programs funding guidelines
- Promote events and activities strategically to ensure optimal participation
- Scheduling of client attendance into the client management system to ensure appropriate charges are applied and data/outputs are reflected correctly
- Support clients to attend and participate in groups/activities based on their individual needs
- Develop strong working relationships with external community organisations, councils and stakeholders
- Liase with transport coordinators to provide clients with transport to and from groups/activities
- Increase and sustain CHSP/HCP clients participation in social groups/activities
- Ensure the secure management of client and organisational confidential information and compliance with privacy policies and legislation
- Provide regular reports to General Manager Client Services
- Identify business development opportunities to support both organisational growth and continuous improvement activities
- Awareness of risk in a community setting, develop strategies and intervention to safeguard clients and workers.
- Develop strong collaborative relationship with other ACS staff / programs and external agencies to deliver seamless and integrated client services
- Incidents (SIRS Reporting), feedback, complaints and improvements reporting to be created in an accurate and timely manner and in accordance with legislation, ACS policies and procedures.
- Undertaking duties where delegated by General Manager, Client Services
- Comply with all ACS Policy and Procedures and Perform all tasks and responsibilities to the level required.
- Assist General Manger Service Development with planning and coordination of major events as required.
